A CLOSER LOOK AT CRITICAL TASK AND HIGH POTENTIAL INCIDENT

HERE IS WHAT CAN AND HAS OCCURRED 

THE IMPORTANCE OF CRITICAL NON ROUTINE TASKS WHEN THEY ARE NOT DEVELOPED THROUGH CRITICAL TASK ANALYSIS, SKILLS TRAINED, AND DEVELOPED WITH JOB OBSERVATION AND RE-TRAINING AS NEEDED. 

THESE TYPES OF TASK PROCEDURES AND WORK MUST BE DEVELOPED WITHIN A TEAM APPROACH TO INCLUDE WORKERS WITHIN THE TASKS, PROCESS MAINTENANCE, CONTROL ROOM OPERATORS, LEADS, SUPERVISORS AND OTHER SPECIFIC KEY PROCESS ENGINEERS AS INDICATED IN ISRS OR OTHER COMPREHENSIVE TYPE LOSS CONTROL SAFETY MANAGEMENT SYSTEMS AND AS REQUIRED BY OSHA FOR REGULATORY COMPLIANCE.
